Each individual participant must complete this form and the acknowledgement of the rules prior to joining a team. To ensure individuals are eligible to participate on a team please be sure to submit this form for our records. HACC 2023 is open only to residents of the State of Hawaii or non-resident students, currently living in and attending school in the State. You will need to provide proof (HI driver license or Student ID) to receive any prizes.
