Each individual participant must complete this form and the acknowledgement of the rules prior to joining a team. To ensure individuals are eligible to participate on a team please be sure to register here first.
Team
Team captains are responsible for submitting a team registration. Please submit only one team registration form per team using the same email address used to complete the individual registration form.
